My Responsibilities after an Organ Transplant

I agree to these new responsibilities to protect my health and transplant:

 Call my Transplant Coordinator right away with any health changes. I
know my immune system is suppressed, and infections are serious.

 Take my medicines as they are ordered each day. I will report any unusual side effects to my Transplant Coordinator right away.

 Keep my lab appointments. I know I will have lab work done at least 2 times each week for the first 3 months. I know I will need to have lab work done at least one time each month for the rest of my life.

 Record vital signs in the log given to me at the hospital, or enter them into Transchart. These include my blood pressure, temperature, pulse and weight. I will also call in with my vital signs on the days I have lab draws.

 Remain active in my treatment by taking my immunosuppressant drugs. If I have a temporary problem being able to pay for my drugs, I will contact my transplant social worker for help 3 weeks before my last dose.

 Not to push, pull, or lift any object weighing more than 10 pounds for the first 3 months after surgery.

 Keep health insurance to the best of my ability after my transplant. If I am eligible for Medicare, Medicaid, or any other health insurance, I will complete an application to enroll in a timely manner. I understand the immunosuppressant drugs increase the success of my transplant.

 After receiving my transplant, I understand these requirements will continue the rest of my life.
